Hannah Pudsey, of Driffield, East Yorks, was due to wed boyfriend Kevin Preston, 24, in July but died last Wednesday.
Hannah, who was born with two holes in her hearth, had a heart op in 2001 after nationwide appeals for a donor.
Her mother Ros said: "The transplant gave Hannah 11 good years she would not have had. She counted every day as a blessing."
She collapsed and died after her heart stopped beating last week.
Ros continued: "Hannah counted every day as a blessing, but she was so looking forward to the wedding. She and Kevin had lots of plans. But instead of planning their wedding, we are now having to organise her funeral.
"Kevin was the love of her life. He was devoted to her. They just wanted to get married and have a family one day."
