The couple were married for four years.
It is understood they have been trying to overcome problems in their marriage, but they will now "move forward" with divorce proceedings.
According to The Sun newspaper, a source is quoted as saying: "They are properly over as a married couple. It's gone past the point of no return.
"Jamie's just waiting for Kate to move forward with divorce proceedings. Neither are in any hurry but they both suspect there won't be a reconciliation. They accept they'll probably have to make some sort of statement but don't want to feel forced into anything."
Moss, 41, and Hince, 46, married in July 2011 after four years of dating. The couple were introduced by a mutual friend, shortly after the model ended her engagement to rocker Pete Doherty. The couple have no children together, but Moss is a mother to Lila, 12, from her former relationship with Jefferson Hack.
